The four-wheel-drive was travelling toward Bathurst when it left the road, crashed through a fence and hit a tree in a nearby paddock.
Ambulance Service of NSW inspector Rhys Dive said the 19-year-old female driver was airlifted to Orange hospital.
"We took spinal precautions and she has severe head injuries, chest fractures and a broken shoulder bone," he said.
The accident occurred 200 metres past Koonong Place.
