Two Commodores - one white, one silver - collided head-on just west of the Beekeepers Inn around 12.45pm.
The white Commodore had been travelling east towards Bathurst and the silver Commodore travelling west on the highway.
Chifley local area command duty officer Inspector David Abercrombie said a triple-0 call came through to emergency services saying there had been a head-on collision 24km west of Bathurst at Vittoria.
Emergency personnel rushed to the crash, but nothing could be done to help the female passenger of the silver Commodore who was pronounced dead at the scene.
Late Sunday her name had not been released.
The male driver of the silver Commodore, along with the male driver of the white Commodore, and his female passenger were all transported to Orange Health Service by road ambulance; all three remain in a stable condition.
Police immediately established a crime scene and cordoned off the road as crash investigation officers from both the Western Region and Metropolitan commands made their way to the area.
“Police and other emergency services all responded to the emergency call just after 12.45pm,” Inspector Abercrombie said.
“Unfortunately a female passenger in one of the cars was killed in the crash.
“The drivers of each vehicle, both men, were transported to Orange Base Hospital for treatment, as was another female passenger.”
Inspector Abercrombie said it was too early to tell what had caused the fatality with specialist crash investigators arriving on scene just after 2.20pm.
With the Mitchell Highway closed, traffic was diverted through Blayney, with the diversion still in place late Sunday evening.
As police pieced together the final movements to determine what had caused the crash, Inspector Abercrombie said the terrible tragedy was a timely reminder to people to take care on our roads.
Police will prepare a report for the coroner.
