The 56-year-old died at the scene of the crash at Vittoria, west of Bathurst, about 12.45pm.
Meanwhile, a 21-year-old man and a 16-year-old girl, remained at Orange hospital on Monday following the collision.
The pair from interstate had been travelling eastbound on the Mitchell Highway in a white Holden Commodore.
The woman from Orange, was the front seat passenger of a westbound, silver Holden Commodore and died at the scene.
Her 56-year-old husband, who was driving the vehicle, was taken to Orange hospital for treatment to minor injuries and for a mandatory blood and urine test but has since been released.
According to police, about 12.45pm on Sunday, a Holden Commodore was travelling east on the Mitchell Highway at Vittoria, when it collided with another Holden Commodore.
The 21-year-old male driver of the eastbound Commodore was taken to Orange Health Service with a fractured cheek and for a blood and urine test.
His 16-year-old female passenger was also taken to Orange hospital, with minor injuries.
Chifley Local Area Command duty officer Inspector David Abercrombie said a triple-0 call came through to emergency services saying there had been a head-on collision 24km west of Bathurst.
Emergency services rushed to the scene but nothing could be done to help the Orange woman.
Police established a crime scene, cordoning off the road to traffic while members of the Crash Investigation Unit investigated the incident.
The Mitchell Highway reopened in both directions at 9.30pm on Sunday.
Police are continuing to make inquiries and a report will be prepared for the coroner.
