Emergency services were called to Mountain Avenue, Woonona, about 2.30pm on Wednesday after the man phoned for help.
Paramedics responded to the shooting, which police believe to be targeted, with a number of resources including a helicopter.
It is believed the 29-year-old was shot in nearby Joanne Street before getting into his car and driving to Mountain Avenue, where he raised the alarm.
A police spokeswoman said the man had "severe facial injuries".
He was taken by helicopter to Sydney's St George Hospital in a stable condition. The injuries are not thought to be life-threatening.
Police remained on the scene and in nearby streets on Wednesday evening.
Supterintendent Mark Wall said a team of detectives were working to locate the offender and establish a link between the shooter and the victim.
"At this stage the investigation is only in its infancy, we haven't made any arrests," he said.
