Duty officer with Chifley Police District, Inspector Glenn Cogdell said the crash occurred at Evans Plains, just after 3.45pm, with both drivers trapped in the wreckage for a short period.
Police Rescue, Fire and Rescue NSW, Ambulance, SES and the Rural Fire Service all attended the crash, which closed the highway in both directions, while emergency crews worked at the scene.
The 18-year-old, who was driving a Proton Jumbuck, which was travelling west, suffered serious leg injuries after her car collided with a Holden Barina, being driven in an easterly direction by an 81-year-old from Gooloogong.
Inspector Cogdell said investigators are still piecing together the cause of the collision, and investigations in the crash remain ongoing.
“The teenager was treated at the scene by paramedics for injuries to her legs, before being airlifted to Liverpool.
“The 81-year-old female, and her 62-year-old male passenger, also from Gooloogong, were also treated at the scene, and taken to Orange Base Hospital via road ambulance for further treatment.”
The incident was the first of two serious crashes in the Chifley Police District on Sunday, after a man was killed in a single vehicle crash near Lithgow.
Emergency services were called to the Great Western Highway at Wallerawang, about 15kms north-east of Lithgow, just after 5pm on Sunday evening after a Holden Commodore towing a boat jack-knifed and crashed.
The male driver and sole occupant died at the scene. He is yet to be formally identified.
Officers from Chifley Police District attended and are investigating the circumstances surrounding the crash and are preparing a report for the Coroner.
