The Australian Transport Safety Bureau released its final report into the May 18 crash on Thursday, where pilot Ben Wheeler, then 47, and Bathurst-based instructor Catherine Fitzsimons, then 55, crashed into grass near the runway while undertaking an "instructional night flight".

Security footage from Orange City Council, which runs the airport, showed the Cirrus SR22 light aircraft hitting the ground after Mr Wheeler became "spatially disorientated".

The report said the rocket-deployed parachute system "discharged uncommanded" nine minutes after the aircraft crashed to earth, with the vision "a reminder for emergency responders to approach aviation accident sites with caution".

While no emergency service responders were injured during the May 18 incident in Orange, the report said there was a big danger for emergency services while dealing with aircraft.

"The post-impact deployment of the aircraft's parachute recovery system highlights an important safety message for emergency personnel and others who attend aviation accidents, to be aware of the potential dangers of an unactivated rocket-deployed parachute systems," it said.

"The mishandling or misidentification of these systems could be fatal.

"Designed to be activated during a serious in-flight emergency, after a collision with terrain any deformation of the fuselage can put a rocket-deployed parachute recovery system's activation cable under abnormally high tension, making it likely to deploy with any movement of the wreckage."

Rocket-deployed parachute systems are installed in most smaller aircraft, with warning about them normally found on external fuselage or the window next to the parachute exit point, but this isn't consistent across all aircraft.
